First Visit Registration Procedure

This page will help prepare you for your first visit and will ensure that the doctor has all of the needed information to guarantee that you receive the best possible care. Being prepared for your initial appointment will help to relieve any unnecessary anxiety you may be feeling.

On this page, we have also included patient registration forms that you can complete online and submit for your first visit. These forms will help us to obtain background information, medical history, and any specific treatments you are seeking. Please arrive 30 minutes before your initial visit to complete the form and to ensure that we have plenty of time to prepare for your visit with your physician.

Please Print And Fill Out The New Patient Forms So We Can Expedite Your First Visit

Please arrive early so that we can complete your initial paperwork.
Also, please bring the following:

Patient's insurance card
List of current prescriptions and over-the-counter medication, including dose and frequency
Information about the patient's medical and surgical history
Recent test results, x-rays, or relevant records

You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call us. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
